Home Care Aides

Primary functions: provide Home Care Aides and/or health-related services to patients/clients in their palace of residence; provide a safe and clean environment; work cooperatively with the patient/client and family; and report observations and problems to the Supervisor.

Licensed Vocational Nurse

Administer skilled nursing care, under the supervision of a registered nurse for patients/clients of all ages in their place of residence. Participates in the coordination of coordinate care with the interdisciplinary team, patient/client family and referring agency. This position requires graduation from an accredited school of nursing.

Registered Nurse

Administer skilled nursing, care for patients/clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/client and their families and referring agency. Assumes the responsibility for coordinating care.  This position requires graduation from an accredited school of nursing, with an Associate or Baccalaureate degree.

Occupational Therapist

This registered professional will provide coordinated care to patients of all age groups. Plans, implements and evaluates patient care to restore or maintain patient well being. Evaluates occupational rolls and occupational performance of patients with physical, psychological ,and or cognitive impairments. Provides intervention to elicit responses. Collaborates with an interdisciplinary team. This position requires a Bachelor of Science in Occupational Therapy degree.

Physical Therapist

This registered professional will provide coordinated care to patients of all age groups. Plans, implements and evaluates patient care to restore or maintain patient well being. Plans, organizes and conducts physical therapy treatment program based on medical referral and their evaluation.Collaborates with an interdisciplinary team. This position requires a Bachelor of Science in Physical Therapy from an American Physical Therapy approved program.

Speech Language Pathologist

A registered professional who provides therapeutic techniques for the rehabilitation of patients with speech language, hearing, oral or swallowing and cognitive disorders. Provides coordinated care to patients of all age groups. Plans, implements and evaluates patient care plans to restore or maintain patient well being. Collaborates with an interdisciplinary team.This position requires a Master’s Degree in Speech Language Pathology from an accredited college or university.
