As a Home Care Aide on our team, you’ll provide compassionate, hands-on support that helps our patients remain safe, comfortable, and independent at home. You may assist with personal care, daily activities, mobility, meal preparation, light housekeeping, and other routine needs based on each patient’s plan of care. Most importantly, you’ll build meaningful relationships with the individuals and families we serve while helping them enjoy a better quality of life at home.
As a Licensed Vocational Nurse (LVN) on our team, you’ll provide skilled, attentive nursing care to patients in their homes under the direction of a Registered Nurse or other authorized healthcare provider. Your role may include medication administration, monitoring vital signs and health conditions, wound care, treatments, and patient and family education. You’ll also collaborate with the rest of the care team to help ensure every patient receives consistent, coordinated care.
As a Registered Nurse (RN) on our team, you’ll play an important role in providing and coordinating skilled nursing care for patients in their homes. You may perform assessments, provide wound care and treatments, manage medications, monitor health conditions, and educate patients and their families. You’ll also collaborate with physicians, therapists, caregivers, and other members of the care team to develop and coordinate care that supports each patient’s individual needs.
As an Occupational Therapist (OT) on our team, you’ll help patients develop or regain the skills they need to participate safely and independently in everyday life. You’ll assess each patient’s abilities and home environment and provide individualized therapy to address areas such as self-care, mobility, coordination, cognition, and daily routines. You may also recommend adaptive equipment or modifications that make everyday activities safer and more manageable.
As a Physical Therapist (PT) on our team, you’ll help patients improve their strength, mobility, balance, and physical function while receiving care at home. You’ll develop individualized treatment plans that may include therapeutic exercises, gait and mobility training, fall prevention, pain management, and education on the safe use of mobility equipment. Your work will help patients make meaningful progress toward greater independence and confidence in their daily lives.
As a Speech-Language Pathologist (SLP) on our team, you’ll help patients overcome challenges related to speech, language, communication, cognition, and swallowing. You’ll assess each patient’s needs and provide individualized therapy designed to improve communication, strengthen cognitive skills, and promote safe eating and swallowing. You’ll work closely with patients, families, and other members of the care team to help patients maintain independence and improve their quality of life.
